Have you tried Advanced Pain Consultants?
It's Nicholas Kondelis' practice.
I've been there since 2014. I'm very satisfied, despite changes out of his control, he finds ways to help.

I don't know about now, but I was on BCBS there from 2014 to Jun 2020. Unless something drastic changed, I'd think he still takes it.
