Guidance For Short Term/long Term Disability?
I’m really getting in hot water at work for poor performance ever since my fibro diagnosis. If you work for a big enough company hopefully your human resources people can guide you in getting the right forms for FMLA and disability. your Dr will need to fill out their portions of the forms. you'll need to fill out a release of information form for the Dr to do so. forms go back to your HR/management.
good luck
Work mu,st be very difficult, but sometimes it keeps you sane. Maybe part time work could be right for you?
